Flap Surgery

What Is Flap Surgery?

Flap surgery, also called periodontal flap surgery, is a gum procedure for advanced gum disease. The dentist gently lifts the gum away from the tooth, cleans the infection and hardened tartar from the roots, and puts the gum back in place. It reduces deep gum pockets and helps protect the bone that holds your teeth.

What Is Gum Disease?

Gum disease starts when plaque and tartar build up along the gum line. At first, gums bleed and swell. This early stage is called gingivitis and can be reversed with cleaning. If it continues, the infection spreads below the gum and damages the bone. This is called periodontitis. Deep gaps, called pockets, form between the gum and the tooth. Untreated gum disease is a major cause of tooth loss in adults.

Signs of Gum Disease

  • Bleeding gums when brushing or flossing
  • Red, swollen or tender gums
  • Bad breath that will not go away
  • Gums pulling away from the teeth
  • Loose teeth or teeth moving apart
  • Pus between the teeth and gums
  • Pain when chewing

When Is Flap Surgery Advised?

The first step is deep cleaning, called scaling and root planing. Flap surgery is advised when pockets stay deep, usually 5 mm or more, and the gums remain infected after the cleaning. X-rays and gum measurements help decide.

How Flap Surgery Is Done

  1. Numbing. Local anaesthesia is given, so you do not feel pain.
  2. Lifting the gum. Small cuts are made to fold the gum back.
  3. Cleaning. Tartar and infected tissue are removed from the tooth roots and bone.
  4. Reshaping. The bone may be smoothed. Bone grafts or regenerative material may be used in some cases.
  5. Closing. The gum is placed back and stitched.

Each part of the mouth usually takes about 45 to 90 minutes. Surgery may be done in sections over several visits.

Recovery and Aftercare

You may have mild pain, swelling and some bleeding for 2 to 3 days. Your dentist may give pain medicine and a mouth rinse.

  • Eat soft, cool food for the first week
  • Do not brush the surgical area until told, but keep the rest of your mouth clean
  • Do not smoke, as it slows healing
  • Use a salt water rinse after the first day, if advised
  • Stitches are usually removed after 7 to 14 days
  • Do not skip follow-up visits

What Results to Expect

After surgery, the gums fit more tightly around the teeth, pockets become smaller, and bleeding usually reduces. Gums may look shorter and teeth may feel more sensitive for a while. Good home care and regular cleaning every 3 to 6 months are needed to keep the results.

Risks

  • Bleeding or swelling
  • Sensitivity of the teeth
  • Gum recession
  • Infection, if aftercare is not followed

How to Prevent Gum Disease

  • Brush twice a day with a soft brush

Frequently Asked Questions

Is flap surgery painful?

The procedure is done under local anaesthesia. You may have mild soreness for a few days.

Can gum disease be cured?

It can be controlled, but it needs long-term care. Bone that has been lost does not always grow back.

How long is recovery after flap surgery?

Most people feel better in a few days, and gums heal over 2 to 4 weeks.

What if I do not treat gum disease?

Teeth can become loose and may need to be removed. Gum infection has also been linked to problems in people with diabetes.

Will my teeth look longer after surgery?

The gums may pull back a little as swelling reduces. Your dentist will explain this beforehand.
