Dental Onlays

What Is a Dental Onlay?

A dental onlay is a custom-made cover that repairs a tooth with damage too large for a normal filling, but not large enough to need a full crown. It covers one or more of the chewing points, called cusps, on top of the tooth. Onlays are usually made of ceramic or a strong composite material and are fixed in place with dental cement.

An onlay saves more of your natural tooth than a crown, while giving more strength and a better fit than a large filling would in the same spot.

When Is an Onlay the Right Choice?

  • A large cavity that is too big for a standard filling
  • A tooth with an old, large filling that has cracked or is wearing out
  • A cracked cusp, where part of the chewing surface is damaged
  • When a dentist wants to preserve more natural tooth than a crown would allow

Dr. Urvi Patel will tell you honestly if a filling, onlay or crown is the right fit for your tooth, based on how much of it is damaged.

How an Onlay Is Placed

  1. Examination and X-ray. The dentist checks the extent of damage and decay.
  2. Preparing the tooth. Decay is removed and the tooth is gently shaped to fit the onlay.
  3. Impression or scan. A mould or digital scan of the tooth is taken.
  4. Temporary cover. A temporary filling protects the tooth while the onlay is made.
  5. Fitting. At the next visit, the finished onlay is checked for fit and bite, then cemented in place.

This usually takes 2 visits, a few days to a week apart, depending on where the onlay is made.

What Are Onlays Made Of?

  • Ceramic. Tooth-coloured and very natural looking, suited to visible back teeth.
  • Composite resin. Also tooth-coloured, generally a more affordable option.

Caring for Your Onlay

  • Brush twice a day and clean between the teeth
  • Avoid biting very hard foods such as ice
  • Wear a night guard if you grind your teeth
  • Visit your dentist every 6 months for a checkup

With good care, ceramic and composite onlays commonly last 10 to 15 years or longer.

Why Choose OrthoD Hospital

  • A clear explanation of why an onlay suits your tooth better than a filling or crown
  • Careful matching of the onlay colour to your natural teeth
  • Follow-up care in the same clinic

Onlay Cost

The cost depends on the material chosen, the size of the onlay, and the tooth being treated. A written estimate is given after examination.

Frequently Asked Questions

Is an onlay the same as a filling?

No. A filling is placed directly in the tooth in one visit. An onlay is custom-made outside the mouth to fit the exact shape of the damage, which usually needs two visits.

Is an onlay the same as a crown?

No. A crown covers the entire tooth. An onlay covers only the damaged part, including the chewing surface, so more of your natural tooth is kept.

Does getting an onlay hurt?

The tooth is numbed before any preparation, so you should not feel pain during the procedure.

How long does an onlay last?

With good care, ceramic and composite onlays commonly last 10 to 15 years or more.

Can an onlay be used after a root canal?

Sometimes, depending on how much tooth structure remains. In many cases a crown is preferred after a root canal on a back tooth. Your dentist will advise based on your specific tooth.
